I think the only lemma on Wiktionary that meets all the criteria (3 syllables, starts with *hl-, contains both a diphthong and a non-intervocalic dental obstruent) is *hlaidrijō ‘ladder’.

Hint, the form that was used in the derivation was not the nominative form. You'll have to search a little bit deeper than just the citation form.
